- The practice is on the 4th floor, Suite 4080, directly above Lawrence Station

Arrive Early for Your Appointment
- Arrive 10 minutes early for your first appointment
- Bring a piece of photo ID and your insurance card (if applicable)
- New patients can complete forms online in advance via the New Patients page
